How referrals work

  1. Send a referral

Use our online referral form (it covers NDIS, Support at Home, TAC and private clients) or call us. Clients and families can contact us directly too.

  2. We're in touch within 24 hours

We confirm the details, the funding and who to contact, then book a time that suits the client.

  3. First home visit

We aim to see new clients within 1 week. The first visit is an assessment and a conversation about the goals that matter to the client.

  4. Ongoing therapy

Physio and/or OT at home, with reports when the funding needs them, and updates to the care team with the client’s consent.

What to include

The referral form asks for what we need. Having these ready saves a phone call.

Questions from referrers

Can one referral cover both physiotherapy and occupational therapy?

Yes. Say so on the referral form, or tell us when you call. Our physios and OTs are one team, so the client deals with one provider.

Do you see NDIA-managed participants?

Yes. We're a registered NDIS provider, so we can see NDIA-managed, plan-managed and self-managed participants.

Can clients or families refer themselves?

Yes. Anyone can use the referral form or call us. For Support at Home clients we confirm the service with their provider before the first visit.

What if my client's suburb isn't on your list?

Call us on (03) 9123 4423. Our list isn't exhaustive, and we may still be able to visit.

Is the referral form secure?

The form goes straight into our practice management system (SPLOSE). Client details aren't stored on or emailed from this website.
